CCSD schools and departments must follow the process below when requesting supplemental instructional technology and student data privacy reviews of software and applications. Requests with missing information will not be processed until all information is provided.

If an application is already on the SAFE List, you do not need to complete a CCF-386 form, nor put in a SAFE List request.

Submit questions to the User Support Services – Help Desk and route to Group: US-SAFE List. If you need assistance submitting a help desk ticket, contact the User Support – Help Desk at 702-799-3300.

Generally, to request that a new app be added to the CCSD SAFE list, a user will need to follow the steps below.

  1. Core instructional material purchase approval Form (CCF-386) – The purpose of the CCF-386 form is to provide details about the application and its intended instructional use. If this is a Staff ONLY application, no SDUQ is needed.
    1. Download and complete the CCF-386 form.
    2. Signatures from the school principal, school supervisor, and the Academic Unit representative are required on the form.
    3. The signed and completed CCF-386 form is returned to the requester and uploaded to the SAFE form (step three).
    4. The signed and completed CCF-386 form is submitted to Purchasing for the shopping cart process.
    5. The CCF-386 form is required for SAFE review and approval.
  2. Student Data Usage Questionnaire (SDUQ) Form – The purpose of the SDUQ form is to provide CCSD with the type of confidential student data collected, how the data is used, and the vendor’s data security plan. If this is a Staff ONLY application or a General Purpose Platform, no SDUQ is needed.
    1. Download and provide the SDUQ form to the application vendor.
    2. The application vendor completes the form and returns it to the requester.
    3. The completed SDUQ form is uploaded to the SAFE Form (step three).
    4. The SDUQ form is required for SAFE review and approval.
  3. Software and Applications for Education (SAFE) Request Form – Schools and departments use the SAFE Request form to request a technology and student data privacy review for software and applications they wish to use as supplemental instructional materials. Additionally, this form is used to request a review of applications for student communications (Policy and Regulation 4100).
    1. While completing the SAFE Request Form, answer the questions to the best of your ability. The form will automatically direct you to attach the SDUQ and CCF-386 forms as needed based on your responses.
    2. Complete all required fields and submit the SAFE Request form.
    3. Required for SAFE review.
    4. Enterprise Student Information Systems (ESIS) team will receive an email notification of the submission. ESIS will then begin the SAFE review process.
