Any application listed on the CCSD SAFE List Website with a requirement of “Parent / Guardian Permission Required” means that the application operates outside of our standard data contracts and a signed Instructional Digital Resources Universal Consent Form is required before any student information is entered into the program.

Please distribute the form to families on school letterhead and file the parent/guardian-signed copies with your school’s athletic department or designated office. Each signed form must be kept on file at the school for one year, as noted on the document. Please find the Universal Consent Form at the URL below:
