Active Directory (AD) Account request form

This form should be used if an additional account is needed for employees (and some non-employees) working at multiple sites such as:

    • CCSD Substitute Employees
    • Non-Employees (Temps/Charter Schools)
    • CCSD Employees in need of additional AD rights at a different location (i.e. second school) Rights request may be assigned by granting a secondary AD account

Pathlore Enterprise Learning Management System and Crystal Reports Access

Please submit this form to request class administration rights and roster access for the Pathlore Enterprise Learning Management System and Crystal Reports. Please note you must complete the Pathlore Fundamentals class and have your site administration approval prior to class creation and administration access being granted.

Note: This is form is not necessary for access to Pathlore to take classes. All CCSD employees are granted class registration access automatically.
